Job Description: LPN - Long Term Care Location: Huntington, NY Hourly Rate: $40 Introduction: We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our esteemed long-term care facility in Huntington, NY.

Responsibilities: - Utilize your nursing skills and knowledge to deliver exceptional care to long-term care residents.
- Collaborate with the healthcare team to develop and implement individualized care plans for each resident.
- Administer medications accurately and monitor their effects.
- Monitor and report any changes in residents' conditions to the supervising Registered Nurse (RN) or physician.
- Assist in planning and implementing rehabilitative and therapeutic programs.
- Provide emotional and psychological support to residents and their families.
- Ensure accurate documentation of resident information, medical history, and care provided.
- Adhere to infection control and safety protocols to maintain a safe environment for residents and staff.
- Collaborate with interdisciplinary team members to provide comprehensive care and promote a positive working environment.
Requirements: - Valid LPN license in the state of New York.
- Proven experience as an LPN in a long-term care setting.
- Strong knowledge of nursing principles and practices.
- Excellent critical thinking and problem-solving skills.
- Exceptional interpersonal and communication skills.
- Ability to work effectively both independently and as part of a team.
- Proficiency in electronic medical record systems.
- Compassionate and patient-oriented approach to care.
- Availability to work flexible schedules, including weekends and holidays, as required.
